These sessions are regarded as very valuable to the student’s learning. However, they need substantial facilitation, specialist equipment, and students from different programmes in the same place at the same time, thus making them challenging to organise.

We wanted to create something flexible, engaging and easy to implement. Simultaneously, we were also interested in a less familiar but very real professional relationship: the relationship between clinicians and those involved in healthcare management, digital innovation and operational decision-making.

In healthcare education, we rightly teach students to think holistically about individual patients and their needs. However, when they enter clinical practice they are working within an environment where resources are limited. Thus their decisions may be influenced by patient flow, bed availability, side rooms, admissions, discharge decisions, infection risk, digital services and financial pressures. Those decisions are not purely clinical, but they shape what clinicians can offer.

From the business perspective, there is a parallel challenge. In digital transformation and healthcare innovation, one of the recurring issues is the communication divide between those who commission or manage digital technologies and the frontline healthcare professionals expected to use them. We saw an opportunity to bring students together early, while they are still learning, so they could begin to understand one another’s worlds, language and priorities. We were all really enthusiastic about that as a concept and recognised its importance.

Our solution (approach)

We developed Scrubs and Suits, a collaborative tabletop simulation game that brings healthcare and business students together to manage a fictional hospital.

We began by mapping the themes where healthcare and business overlapped. Several ideas emerged, but we focused on healthcare management, resource allocation, digital innovation and telehealth. We wanted the activity to be fun, accessible and screen-free, not because digital technology was unimportant, but because we wanted students sitting around a table, talking to each other, negotiating and making decisions together.

The game was designed as a collaborative experience rather than a competitive one. Students work with each other against the challenges posed by the board. In interdisciplinary groups, they take responsibility for running a simulated hospital, making decisions about patient care, departments, budgets and service improvements. They are required to balance individual patient needs with population-level thinking and system pressures.

The early prototype was deliberately low-cost. We created patient cards, department cards and colourful materials that could be printed, tested and adapted quickly. At one stage the design was more complicated, but playtesting helped us strip it back to the essential learning. We focused on patient journeys and episodes of care, because that gave students a shared language at the boundary between clinical and management thinking.

Telemedicine was designed to be an important feature of the game. Students can decide whether or not to invest in it and then experience the benefits and tensions that come with that choice. This decision opens up conversations about why digital care may benefit patients, for example by allowing them to stay at home and reducing some hospital risks, while also raising questions about funding, adoption and how value is recognised in healthcare systems.

We tested the game ourselves, played it with colleagues, and refined the content before using it with students. The final activity is deliberately playful, but the learning is serious. Students encounter ethical dilemmas, operational pressures and competing priorities in a safe environment, then use reflection and discussion to unpack what happened and why it matters.

Scrubs and Suits video

The impact (results)

Student response has been very positive. Across the quantitative feedback gathered so far, the activity has received consistently high ratings, around 8.6 out of 10 satisfaction rate. Students have particularly valued the chance to work with peers from a very different discipline.

The strongest qualitative feedback has been about engagement and insight. Students have described the activity as fun and engaging (one student said ‘it was ‘the most engaging and fun learning exercise that they’ve had’), but also useful because it helps them understand how different professional groups approach the same problem. Healthcare students gain insight into management, resource allocation and digital transformation. Business students gain a more grounded sense of the clinical pressures, ethical dilemmas and patient-centred realities that sit behind operational decisions.

Students playing the Scrubs and Suits game

The activity has also generated interest beyond the initial student cohorts. Colleagues in the Business School responded positively when the game was shared, and the team presented it at the International Enterprise Educators Conference (IEEC) 2025 in Manchester. We got a lot out attending the conference; having only attended science-based conferences before, it was an exciting experience for us, and it was fantastic to see the reaction to the game from a diverse audience, including professionals, the NHS, business owners, startups, and enterprise education. This wider response suggested that the approach has value not only for students, but also for educators and healthcare professionals interested in simulation, leadership and collaborative decision-making.

One of the most rewarding aspects of the conference was winning the award for the best session, which was a big achievement for us.

Scrubs and Suits recognised with a Best in Track award at the International Enterprise Educators Conference 2025

Scrubs and Suits has since been recognised as an innovative cross-faculty teaching practice and is continuing to develop. The team is exploring how the game could be adapted for different healthcare settings, including GP practices, pharmacies and hospital trusts, and how it could be used with other learner groups. For example, we are working with colleagues in civil engineering to design another version of the game for them, which is moving into the testing stages.

Interest outside the University has also continued. The IEEC got in touch again as they wanted to develop a conference around the game, which became a national playful learning conference hosted by Newcastle University Business School in July 2026. The game has attracted interest from NHS Trusts and the Northeast Simulation and Human Factors Group. It also led to a SimTech podcast appearance. It was nominated for the Patil Teaching Innovation Award at AMEE – an international medical education conference that took place in Vienna in 2026.

Bas and Rebecca attending the AMEE conference in Vienna

Lessons learned

The biggest lesson was to keep returning to the learning purpose. It is very easy for a game to become too complex, especially when the topic itself is complex. Our early ideas included more detailed scenarios and more mechanics, but the game worked better when we simplified the structure and focused on the conversations we wanted students to have.

We also learned that low-cost prototyping is a strength. Printing cards, testing them quickly and changing them after discussion allowed us to build something useful without needing a large development budget. A polished resource is helpful, but it does not need to be perfect before colleagues and students can help you improve it.

The analogue format mattered. A screen-free board game encouraged students to face each other, listen, negotiate and make decisions as a group. The fun of the activity helped students engage with topics that can otherwise feel abstract, dry or intimidating.

Finally, the debrief is essential. The game creates the experience, but the reflection turns that experience into learning. Students need space afterwards to identify what they noticed, what surprised them, where they disagreed, and how the activity relates to real healthcare practice.

Tips for colleagues

Start with a real boundary problem: an issue where two or more disciplines need to understand each other better. The strength of Scrubs and Suits comes from the fact that healthcare and business do not always meet during university study, but they absolutely meet in professional healthcare settings.

Prototype quickly and test early. A simple version made from cards and paper can tell you a great deal about whether students understand the task, whether the decisions feel meaningful, and where the activity is becoming too complicated.

Make the activity collaborative. Asking students to work together against the pressures of the simulation encourages communication, negotiation and shared problem-solving.

Keep groups small enough for everyone to contribute. Groups of around four to six students worked well because they created enough diversity of thinking without making it easy for individuals to disappear.

Build in a structured debrief. The most valuable learning often happens when students look back at the decisions they made and consider what those choices reveal about professional roles, ethical priorities and system constraints.

Skills and attributes

Students were able to develop the following attributes:

<a
