Health and Safety incident reporting system

Due to the ongoing IT issues the Incident Reporting System is currently unavailable.

In the meantime please could you ensure that incidents (accidents and near misses), fire incidents and first aid reports are completed via the word document forms that are available here.

These should be submitted to ohss-safety@ncl.ac.uk.  Your lead Health and Safety Adviser will respond to these as required and also forward reports onto the relevant managers within your Unit in line with the Incident Management Investigation Guide.
